Our Self-directed services help people plan their support and services in a way that meets their individual needs and helps create a good life, while optimising funding and community inclusions.
At Mercy Community Services (MCS) we utilise our existing networks and strong community partnerships to assist people to maximise their support to meet their individual needs and desires.
A skilled worker will assist individuals and their nominee/s to create a holistic support plan which identifies client’s goals, dreams, needs and how they would like to use their support services. Individuals are active participants in designing their support plan, and MCS will work with them and their family or carer to identify resources to plan, purchase and select the services that will best meet their needs.
We understand it can be difficult knowing where to start and being expected to be able to articulate your goals, dreams, support needs and structures in one session. Therefore, we have built our service around supporting people through this process by:
